We use an approach called person-­centred planning, which puts the people we support at the heart of their support. We use a variety of tools to make sure people can exercise choice and control in their lives.

Person-Centred Support Tools

One Page Profile

Everyone who works at Joymac or is supported by us has a One Page Profile. This summarises who you are as a person, and we encourage you to create one too. This helps everyone to understand what is important to a person and how we can work well together. It also enables us to match the people we support to the staff who support them.

Planning Live

This event gathers everyone involved in supporting someone to create a personalised support plan. Together we look at your relative’s goals and ambitions, what’s important to and for them and how they like to spend their time. We use this to create personalised support for them based on their goals and ambitions.

My Support Plan

Helping shape and focus the support we provide to your relative, My Support Plan is regularly updated at person-­centred reviews. It outlines what your relative wants from their life and support, and how we can support them best. My Support Plan is used by everyone involved in your relative’s support.

Person-Centred Reviews

Through person-­centred reviews, we look at what’s working and not working with your relative’s support, working together to decide how things can improve. We use these reviews to update your relative’s support plan with new developments, goals and needs

Staff Appraisals

Your relative’s support plan shapes staff and team objectives. To help make sure our staff provide good support in the right way, each year we may ask for your feedback for their appraisals. This is an opportunity to compliment their work, or raise any issues you feel need focusing on.

Staff Recruitment and Matching

At Joymac If we’re looking for a new member of staff or team to support your relative, you can get involved in staff recruitment or matching.

As well as having the right skills and aptitude, we make sure our support workers connect with the people they support, and have similar interests. This is important to help with building a trusting and productive relationship. To do this, you can get involved in their recruitment. You can help write job descriptions, interview candidates and see how they interact with you and your relative. This helps to direct who we recruit to support your relative.
